What is the NHTD Waiver?

The Nursing Home Transition and Diversion (NHTD) Waiver is a Medicaid-funded program in New York State that supports individuals with physical disabilities or seniors who require a nursing home level of care. Instead of remaining in institutional settings, eligible participants can live safely at home or in the community with tailored support services.

What is the TBI Waiver?

The Traumatic Brain Injury (TBI) Waiver program provides specialized services to help individuals diagnosed with brain injuries maintain independence and community involvement. Who Qualifies for NHTD & TBI Services?

To participate, individuals must:
  • Be a New York State resident
  • Be Medicaid-eligible
  • Require a nursing home level of care
  • For NHTD: Be 65 or older, or 18–64 with a physical disability
  • For TBI: Be 18–64 with a documented traumatic brain injury
  • Be able to live safely in the community with available supports and services
